The Practice List is currently open. 

An application for inclusion on the list shall be made by delivering to the Practice either a medical card or a completed HS22X/HS200 form (if a medical card is not available). These forms are available at reception.

As part of the registration process, you will be asked to make a new patient appointment with a GP/Practice Nurse. You will be given a new patient questionnaire to complete which you should bring back completed on the day of your appointment. This questionnaire is confidential and is used to identify any previous/current medical history/conditions that you may have. The Practice Nurse will assist you if you are unsure about any of the questions. It is essential that an up to date vaccination history must be provided for a child wishing to register with the Practice.
